COVID Vaccine:

To make a vaccine appointment at a state or pharmacy run location, you may follow the link below. At this time, our office is unable to schedule appointments for you as you will need to complete a self-attestation to book your vaccine. You will NOT need a doctor’s note or other documentation to show you have two or more of the above conditions.

Vaccine Appointment Finder:

If you are eligible to receive the vaccine, we encourage you to visit mass.gov/vaccine to find a location near you and to make an appointment. If you do not have access to the internet or if you are having trouble using the internet, you can call 2-1-1 for assistance with scheduling an appointment.

Pharmacy Based Vaccination Locations:

In addition to use of the state-managed vaccine scheduler, eligible individuals can now book vaccine appointments directly at pharmacies via each pharmacy’s online scheduler as below:

You do NOT need to confirm your eligibility for vaccination with your physician; however we are happy to discuss this topic with you if:

  • You are under 65 years old and have questions about your eligibility due to medical conditions

  • You have concerns or questions about getting vaccinated because you are pregnant or breastfeeding, due to certain medical conditions, or for any other reason.

What to expect if you aren’t currently eligible for vaccine:

If you do not qualify for a vaccine at this time, there are no steps for you to take right now. Please visit mass.gov/vaccine to learn more about the phases and to find additional information about the COVID-19 vaccine.
